Applying from Japan - Police Check and Medical Report Question

I'm applying to sponsor my wife (we live in Tokyo and will be moving back to Canada in 2015).

The Manila office are very difficult to get a hold of, and I just need a couple of documents before I can move on with the process:

• In regard to the medical report, there appears to be a document called IMM 1017 that needs to be submitted. I can't find this form anywhere online. Is this provided by a physician? What is the medical report called in Japanese? Will any kind of medical exam do? Nothing I've read get into specifics about this.
  
• What does the police/criminal record involve? As far as I can tell, my wife needs to apply directly at the Consular office in Tokyo. Is this accurate?

Medical has to be done by a Panel Physician.I seem to remember you need to take Appendix C with you to appointment, but I can't find online either.
Approved Physicians for Japan are listed under country specific guide.

IMM1017 is provided by the Physician after med done. Is the proof to CIC that meds done, send with application.
