As per my knowledge, it goes to Medical Officer (MO) who sits in New delhi. But you never know if they have changed the process, let's wait and see if Umer211 quote the exact wording here.

You will ask the university to defer ur admission to next session or ask them how late can u join classes. Some universities allow late registration and some Donot...On the other hand ther are cases where CHC has issued visa after the commencement of classes...so it is basically universities call, if they allow then join late or otherwise defer...

1. Could you quote exact words? because my medical letter says: "Note to Doctors: Return all results directly to: Health Management Branch, Canadian High Commission, ....New Delhi-110021, India.
2. Medical Officer sits in New Delhi.
As Quoted on my Medical Form:
" The medical examination must be performed by an IOM Canadian Health Assessment Panel Physician. The results of your medical examination will be forwarded to a Canadian Medical Officer who provides an opinion on your medical condition to a Visa Officer who will examine your application. The IOM Canadian Health Assessment Panel Physician cannot provide any further information related to the processing of your application.
Certain medical conditions may require further investigations and you may b referred for additional tests. These tests are part of the satandard medical exam and are based on guidelines set out by the canadian Medical officer. The IOM Office will forward all medical results directly to our Medical Officer at the CHC, New dehli.
