Appendix "A" how to complete it? is this a form? neeed help please.....

after medical examination......just recently my niece receive a letter instructing her to send her original passport, picture and Appendix A but we don't know what is that Appendix "A" as I red the letter it seems it is a form but no attach form included in the letter... so please help if you knew this thing thanks.....

Appendix A - IMM 1017 - you get it from the DMP when he/she completes the medical.
You do not complete it, the DMP does.

But WHY would they request the applicant to send IMM 1017? Or I wander what the exact wording of the letter from CIC is..
