It is quite possible considering that off late a few people have had background change BEFORE medical based on tracker info. I guess with CIC, anything is possible!
After they did 3 back to back draws giving away over 10,000 ITAs in 3 weeks, I have realized that ANYTHING is possible with CIC! And when I was all ready to get my ITA, I had to wait for 3 weeks since that's when they decided to take a break for 3 weeks! :-X

This calculator is based on average trends where people used to get it at an average of 30 days from past data. However if you keep scrolling down the list, you can see so many people who have crossed 45 days and still no medical pass.

Not Necessary to have picture. The one give by the Indian Embassy doesn't have a picture. However when they embasssy gives the PCC, they do stamp the issuance of the PCC in the passport and that stamp page needs to be uploaded in the Passport scan. I have no idea about non embassy PCC.
Depends on where you live. If you live in India, you get it from India. if you live in Canada, you can get it from the Indian Embassy in Canada. If you get it from Canadian embassy, it will be valid till you go back to India next. If you live in Canada but decide to get it from India, you will have to ensure that you get your ITA and AOR in 6 months time.
