Yes, it is very common that a schedule A request is being made to applicants, usually it means they'd like to know more about a certain period of your background history don't worry.
In your application page, if you go to the section where has a list all your transmitted documents, you should be seeing an additional row for schedule A upload. Simply prepare the document like you did for other ones, scan and then upload there.

I actually have no idea on the "always" part, not everybody will be asked for it. My speculation is that if you're previously rejected any visa/government document application, or if you didn't provide enough detail on your application of what you're doing for a particular period in your history, or if you have very frequent travels to many different countries, chances are you'll be requested for schedule A very high.
For me as PNP -applicants, schedule 4 is mandatory but I have not received schedule A request yet.

April 14th - request for Schedule 4 - Prov Nominees: A newly completed Schedule 4 Economic Classes – Provincial Nominees form (provided the same day)
April 15th - medical changed to "Passed"
April 18th - request for Schedule A: A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature (provided the same day)
If someone wants to know why they ask for schedule A so often - the form changed since last year, that's why

how many years of elementary school you have completed in the first box (for me it was 8)
how many years of high school you have completed in the second box (for me it was 4)
how many years of college/university you have completed in third box (for me it was 3.5, but WES assessed this as 3 year degree, so I put 3)
how many years of post-secondary (non college/university) you have completed in fourth box (for me it was 1, cause i have post-graduate studies which was not recognized by WES)
