Annex A Questions

I have received PPR and at the end of the PPR email there is a form, called Annex A which basically asks for your Canadian or US address and they have asked me to attach the Annex A with my passport and photos and send those to CPC. I have 2 questions regarding Annex A -

1. Is the Annex A only needed for those who reside in US or Canada? because the form says 'My Permanent Resident visa and/or confirmation of permanent residence will be mailed to the below Canadian or US address.'

2. The Annex A does not have any field for country (only name, address, zip code, city, apt #), so how do I write the Country name?

The Annex A is just to confirm you are agree mailing to those addresses

I agree that the permanent resident visa will be affixed to my passport and dependents’ passports, if applicable.


My Permanent Resident visa and/or confirmation of permanent residence will be mailed to the below Canadian or US address.

you could write beside on state/province space
the annex A is just you agree to send your documents to those addresses

Mon visa de résident permanent et/ou ma Confirmation de résidence permanente seront envoyés à l’adresse Canadienne ou Américaine suivante
