CPCO Final Request_Forgot to sign in Annex A

Hi all,

I received my CPCO Final Request email yesterday and immediately sent in my passport with 2 photos and a part of Annex A to CPC Ottawa. When I opened the email, the message was clipped so I did not see the full Annex A form and thus, had only entered my name and address. Today when I opened it again I realized that the full form required me to sign and date at the end. Should I send the complete Annex A to Ottawa now or do I wait for my passport to be sent back to me? Do you know if they could make a note on my case and wait for the complete form to arrive?

I think you might need to send it again but your passport os with them hmmmmm. Any thought ppl? im sorry I've never experienced this.
