I was nominated through job offer and i only applied PR no work permit. I got my medical now and also officer requested Employment records: An original and recent letter of employment indicating the start date, annual or hourly rate of pay, and regular hours per week.
I am confused do they need updated letter of employment from my employer in canada or current employer in pakistan

As per my understanding they are asking you from your current employer in your country of residence or home country . Since You already have submitted such a letter with your IRCC application and for the record update purpose they need the latest .
