What was the reason for the rejection? Was the reason he did not submit an employment reference letter? I thought if someone does not have the reference letter they could should pay stubs + offer letter and additionally getting a no reference letter email or something like that

Yes, he did not submit a reference letter. I believe if someone couldn't obtain a reference letter, they should write a letter of explanation stating that and also include any attempts to obtain such a letter (such as an email correspondence with the employer). This along with pay stubs and offer letter might suffice. I think this individual was misled by his agent into thinking that just pay stubs and offer letter were sufficient. He didn't even know that reference letters were mandatory until he got rejected.
