I submitted my wife's passport for stamping today but had a question. In the last page of the consent form where it says "Applicants wishing to provide biometrics/Applicants NOT wishing to provide biometrics", I left it all blank because she already gave her biometrics a month ago and filled up the same form that time. Basically the first page is filled and signed with all her info. I don't think it needs to filled and signed if we are only sending the passport out for stamping.
