Maybe because they came from a country where a visa is needed?
If you don't need a visa and if you don't work in medical or something with children you don't need to pass a medical exam.
Federal-level processing time is not controlled by Quebec. Quota is controlled.
Lets hope they continue to process 2 months at a time, us 2021 people still may have a chance! In October they should process Nov & Dec. November they should process Jan & Feb 2021 and December Mar & April 2021! We will not get the PR this year but there is a chance for AR this year at least.

Hi Bro, I understand that ur the main applicant and inland, right? If yes then IRCC might have taken into account any previous medical results for yourself. If ur outland, then send IRCC a webform and explain the whole thing and am sure they would reply back.

Yes, my immigration lawyer asked me to wait for few days to see if there is a request coming in. Otherwise contact IRCC.
