I found the eMedical in my e-mail, however it is just the information sheet showing my INFO and UMI (I believe the doctor did not return this paper to me after the consult).

Resubmit your e-medicals again.... Provide your UCI/FILE NUMBER in the report. Good luck.

How can you resubmit the e-medicals? Do you have the e-medicals filled by the Doctor? or just the Information Sheet?

The information report you received from the doctors office after your medicals need to submit.... via GCKEY or WEBFORM.

But I did the medical reports 3 years ago, do you think it still valid? The problem I do not have the medical exam results, I just have the medical request paper
