*Completed package = Your Current & Valid Passport, Passport submission letter received from IRCC, Courier & SMS form duly filled, Consent form duly filled. (https://assets.ctfassets.net/xxg4p8...f3eaf210cf115e35a9bfd096b/VFS-ConsentForm.pdf )

This consent form does not allow me to enter to my details.

You need to download it and fill it using PDF or you can convert it to a Word doc and then fill it either by printing it or on Word itself.
