Since then nothing, tried to contact NDVO through web-form & Emails but not a single word from them.

How did you see update on GC key when it’s giving bad getaway error message?

I got the same error on my computer but it worked on my phone.
It worked twice but reverted back to Gateway error on phone too.

After receiving Pre-Arrival Letter on Nov 4, 2020, I got an update on GCKey saying Medicals was passed (Nov 10, 2020). This went back to standard 'Medicals not needed' in the same day. Now, Dec 1, 2020 again got an update on GCKey saying Medicals passed on Dec 1, 2020.

What do these changes indicate?

Original medicals has expired on Oct 12, 2020.

And why its took so long for your file .. you are 2018 applicant ... normal time is 12 months but in your case so long what happened

I had a review required on EE as I had submitted the reference letters in the wrong format. Submitted the rectified letters in Oct 2019. File was cleared in July this year.
