What is the format for separate sheet you attached?

I added a table with three column
Name in English, Name in Native and relationship.

Of course, some formal writing too like "Respected officer, Here I am ......"

we used for signature only blue in 2016-2017
black is confusing

I need to attach an additional page for SECTION - 3 of IMM 540 (Additional family information form) for my mom, do I need to get this additional sheet to be signed from my mom , or signing the actual form in itself is sufficient?

For IMM 540, some of the family members doesnt have email-address, do I have to write N/A for this or leave it empty?

If I write N/A for email address fields, then i ll have like 10 N/As of that one page form.

Hey guys kind of urgent. I am unable to validate form Schedule A and reason its saying null value in signature. But How can we do signature without printing the form?

And when i click on Application received, then

1. We received "Name of Principle applicant"'s application for permanent residence on April 30, 2019.

So you enter your UCI number. What about Surname, Date of Birth and Place of Birth? Do you enter your information or the Principal Applicant?
