hi friends,
I am applying for my mom only ,
My father passed away and my mom dont have dependent child , as my sister is 30yrs old and living with her husband, My mom have 2 child me and my sister no other dependent.
Q) So that do we need to fill out additional dependent/ declaration form.

In the financial evaluation form 5768, question 6 - it says enter number of other dependents and provide details on the back of this page. I entered the details in question 12. But do I also need to write something at the back of the page also?

If you print application back to back then Q12 will be on back page of Q6..

That's right the wording refers to it in the past because usually people would not apply and co-sign at the same time.

For example, A and B are married, no children.
in 2017 A applied to sponsor her parents. B co-signed. A family size includes only her family + 1 (B).

in 2018 B applied to sponsor his parents. A co-signed. B family size includes everyone because of previous 2017 undertaking. A's family size in 2017 application just changed because she's co-signing now, so she'll have to update IRCC with her new family size.

In your case, both things are happening at the same time, so you start your applications with the larger family size.

No, only immediate family members. Below, inside the table I stated that the family size does not include the parent been sponsored.

which form are you referring to? I believe you have to include sponsored in family sized for each year you declare your income and employment history.

Question - in form imm5768 I have following questions

1. In question 6 we put count of children we have and write their details in question 12 ? Correct

2. For employment history I have written recent 2019 also but I have left income column blank . Is that ok ?

3. In one company I have worked twice in same year . Means i left and rejoined however the same company in same year with gap of 4 months . t4 received from
This company is one so how should I breakdown the income for these two periods

i put the estimated income from Jan to May 2019.

Pls can anyone help me to my question on IMM0008 under the Singapore passport detail for my mum. Do u split the surname & given name on different line as Singapore passport, the name are together in one line unlike other country passport where there is a split of Family name/Given name. But the IMM0008 form stated that name has to be exactly stated on yr passport.

The problem with just one name on the passport, common in Indian passports:

I will be writing not applicable and explaining it in the explanation letter saying i had to write it because it was a mandatory and won't let the form validate otherwise. This is what i would do, but yeah i haven't had any direction from others on the forum regarding this issue.

Even I do not have parents last name in passport but I have put it in the form. So that it is same in all the forms I am sending to CIC. I did the same for my PR card renewal and citizenship applications and there was no issue.

For IMM5768, Q14 I filled like the following

2019-01-01 Present Company: ABC Gross Income : "Say $10000 (Jan to April)" Family Size 4
2018-01-01 2018-12-31 Company: ABC Gross Income : $55000 (NOA line 150) Family Size 4
2017-01-01 2017-12-31 Company: ABC Gross Income : $55000 (NOA line 150) Family Size 4
2016-06-05 2016-12-31 Company: ABC Gross Income : $35000 (NOA line 101) Family Size 4
2016-01-01 2016-06-04 Self Employed Gross Income : $1000 (NOA line 135) Family Size 4

In 2016 June 5th I started with Company ABC , still working and before I was Self employed (sole proprietor). So I separately entered one for ABC and other for Self employment. For 2016 gross income for ABC I just used the Employment income (Line 101) and for Self employment used the Business income (Line 135)
I am sponsoring my father and mother, my wife is co-signing. So for family size I entered 4 (Me+Wife+Father+Mother)

Please advise, the above are correct or not.
