Hi Everyone,

In form 5406, they have asked to fill all names in native language.

Does it goes in same line like "Harendra, हरेन्द्र " or native name need to be in different line. Also do we need to fill all address and everything in native language as well.

Also i am not able to use pdf to enter native language

If it fits you can do in the same line.
If you can't enter native script in the PDF, print the form, fill it by hand.

I am working on my parents application. I have two questions:

1. My husband (co-signer) - is self employed and files income under personal taxes as well.
I should be using the income to fill the forms I see under his NOA line 150 - Correct?
Is it similar for both employed and self employed persons?


2. If I am sending my parents passport copies - do all these need to be notarized even if the passport is in English?

1. My husband is self-employed and I worked for an employer. We applied for PGP 2018 and we are approved as sponsor. (I co-signed his application for his mom)
We both use income stated in line 150 of our NOAs.

2. No need for notarized copy if document is in English. Just plain photocopies.

Hi everyone,

I am having trouble with IMM 5669 question 3. I nor my father knows his fathers date of birth. CIC say to put * if the date is unknown but when I do I still can’t validate the form. I have tried a lot of combinations ****-**-** which is the format for year/month/day and still no luck.

Does anyone know what I can do? Does the form have to be validated or can I send it in not validated because of the date of birth unknown?

There's a note on the guide regarding this situation:
"Note: If you do not know the complete date of birth or if the date is in the 1800s, please use a “*” (star sign/asterisk) to fill in the spaces for the unknown year, month or day. The missing information can be added by hand however the form cannot be validated."

Nooooooooo, just include more than 6 months (which means any permanent resident status other than the origin country)

Are you sure, Some other person said we have to fill all travels even for 2 months also.
I just had to redo the Schedule A for parents regarding this.
Please confirm if we need to include travels less than 6 months or not ?

I have couple of question regarding the application (various forms)
1) Background / Declaration Form -
asks for principal applicant and spouse (parent's) - about there parent's date and place of birth (which is essential date of birth for sponsor's grand parents .. potentially early 1900s). For place of birth - Can we put Indian sub-continent? since than it's divided into 3 countries. Geography of the world has changed in 100 years? or does it have to be current country?
2) Additional family information
if the date for birth are unknown can it be left blank?.

Ah, finally a question on place of birth for people from the Indian sub-continent pre-1947
My mother, now living in India, was born in Lahore in 1940....
I put "Lahore, India" when there is a pull down,
OR
"Lahore, Undivided India" when there is a write-in field.
My logic is that you can't have been born in a country that didn't exist at the time.
I guess this applies to a bunch of people, e.g. former Yugoslavia, former Soviet Republics etc.
BTW, her current passport, issued just a couple of months ago, says "Lahore, Undivided India".

Thank you! I don’t want to sound dafted but on the document checklist it states that for the to be accepted it must be validated. Seems contradicting. So confused.

Forgot to mention that when I do use the * it’s tell me that it’s an invalid character.

Same out consultant did for my parents super visa.
But recently i checked on their website it says do not put “n/a” in the name fields.
So i am confused.
