What does a 5-year multiple entry TRV mean? it's usually valid for 10 years. Anyways, how long can they stay each time in Canada? If it's 6 months, Can they stay longer if they want? should they extend? How do they extend the 5-year validity?

Means exactly that a visa valid for 5 years of multiple entry into Canada as a visitor or temporary resident. Is not a guarantee of entry that will be decided each time by CBSA as will the time anyone is allowed to stay . Could be 6 months could be less.
General advice here even with a multi entry visa is to spend same time outside Canada as in Canada although there is no firm rule for that just more convincing that someone is not trying to live in Canada as a visitor.
Once in Canada there is a process to extend stay as a visitor. Cannot extend the 5 year visa, would need to apply from scratch again for a new visa,
