Does anybody have an affidavit format that can be used in place of a birth certificate? I need it for AIPP and will be supplying my educational documents as well. Thanks

Affidavit will not work, You will need to get in writing from your Municipal Commissioner office.

You mean the "records not available certificate"? They're not willing to give out easily. What should I do in this case? Congrats on your smooth turnaround!
