Hi guys I am filling my citizenship application form. I am in employment history section. I worked in construction for couple of months and my employer gave me a t5018 form instead of t4. I did my research and found out it is given to sub contractors. So does that makes me self employed? So will I give his name as name of employer and contact info or mine?
Question 11 says if you were self employed you must provide the details of the self employment. What details I have to provide for self employment ?

Technically, yes, but I would still put the employer name and contact information there. They wanted to know what you did for self employment. Listing the client fulfills that requirement. (I'm a self-employed contractor and I just listed my client.)

Just to clear up I don't need to attach any additional details for self employment in the employment history other than filling out the employers contact info.
My followup question Am I require to have a sub contractor license or some sort if they want to know details just in case I am asked later. I was assuming I would get t4 but my employer gave me t5018 worked for couple of months in construction.

No additional documentation needed. I would just be prepared to answer questions about your employment at your interview. It's not something to worry about.
