Need to fill and send IMM 5475 form ,
In the section 3 ,

If you have already submitted the application , ***** FYI : I submitted the online application *******

Name of the Office you have submitted the application : ????
Location of the Office : ????

What needs to filled ??? how do we find those details

My husband is the primary applicant and Im dependent . Can I refuse to mention no on in the designated individual Place holder ??? or can spouse details be furnished ???

What office did the app go to? like is it in Canada or other country, they just need to know where was it sent coz its usually 2 steps for sponsorship, first its approval in Canada, then goes to designated country so mention where its now, u must know where its now? first stage, 2nd tsage

Please call CIC (+1 613 -944-4000), and confirm your office location ..

Mostly it would be Sydney ,Nova scotia if your application is in initial stages , remaining details fill it and send

What did you chose to do on the Spouse for the declaration? During filling up the EE profile, i had put my spouse as the designated individual but later removed her during ITA submission so as to avoid any additional paper work..

I have got the same query now. Were you able to locate the office through the call centre?

Most immediately after you submit your online application , it goes to office in Sydney, nova Scotia . When i contacted the call center , they told the same . Hope it helps

I too got this request from CIC to submit the 5475 form and I'm not ab;e to get in touch with CIC call center person. I choosed address change option and its keeps on waiting.

Also if Location of office is Sydney, nova Scotia, what will the Name of the Office you have submitted the application : ????
