Regarding GCMS notes I think you have enough information to start off, we dont even have such information in our GCMS, just try to address them in advance. How did you get married in Pakistan, was it arrange ? Have you visited Pak regularly in the past, try to gather as much proofs to show your marriage is geuine and update them.

Is there any connection between lvo visa officer and medical request ? Bcoz lvo asked me two times additional documents... after submitting of add docs , now i recieve medical request ... is it means vo is satisfied about my application papers??

Are you people know each other before marriage? Or it was a love marriage type.?
You said your nikkah and rukhsati done from home. That is part of culture, some people do it at home some at masjid.

I hope you explained to them. You should have mentioned that in our culture these kinds of trends are common
