You the principal applicant have been issued a civil union certificate ?

I should mark it to true or false on the check list?

We did not check this. We only checked the one above it. If you the PA are married to the sponsor, provide marriage certificate/registration.

If you courier it, you'll be able to keep track of your application, and if they don't deliver within the specified time frame, you'll be able to get the money back. You don't want to lose the application! If I were you, I will make a copy of the entire packet before mailing it.

Civil Union certificate - legally recognized union of same gender couple. This is not applicable for you, or anyone else for that matter (at least in this forum).

civil unions are also common law unions. Not necessary same gender marriages.
