It is difficult. You need a job offer and your employer needs to file for and pay $1k for a LMIA. The LMIA shows that no Canadian or PR could be hired. Then with LMIA, you apply for a closed work permit. For the permit, you show that you are experienced to do the job. The process can take many months. So start applying for jobs on Canadian job sites. Most postings will state only if you are authorized to work in Canada so don’t apply to those.

Thank you very much for your response. Does this mean you can't be eligible for express entry when you've never been in Canada and don't have any relatives schooling or who's a permanent resident?

You don’t need a job offer or relatives to be eligible for Express Entry. Most people who are invited don’t have a job offer or family in Canada.

I've got all the other criteria for CRS score but I got the feedback that my CRS was low and need to improve it by getting a job offer, being accepted by provincial nomination and other factors which indicated I at least need to have a connection in Canada

Even though I was eligible for the skilled worker program, express entry saw my entries to be ineligible and they sent the reason for that

You are not eligible as your language must be CLB 7 in all areas. 5.5 is too low. What about your ECA (Degrees)? If doctor, your ECA must be completed through the Medical Council of Canada. If not completed then you are not eligible.

Are you aware that it will be extremely difficult to get relicensed in Canada? You will need to pass your licensing exams and then try to secure a residency and then repeat your residency. The big issue is there are very limited residency spots after Canadians students have received their residency spots. If you come to Canada you must be prepared to never practice medicine again. There are other countries that are much better when it comes to immigration for IMGs. The UK is one example.
