Wrong family name on IMM 0008 and on IMM 5406

Hi every one
On aditional information form and on IMM 0008 , I have wrongly written my wifes family name , I have received my AOR I am Jan 2013 applicant .
So could any on pls tell me what should I do
I can print out the family info form with the family correction but what about the generic application form IMM 0008
Should I just print out the page that I nead to correct ?
Or correct the page validate the application and send it ?
All advices are welcome , I know it is a bad mistake and I nead to rectify it so kindly healp.

Did you send the complete form (original) or just the page with correction? and did you receive any confirmation from CPP-Ottawa confirming they added the corrected form to your file?

I did send a new form with all the correction needed and a explanation letter.

was your new form attached to the old application set. did you recieve any acknowledgement from them. was your case accepted
