Work permit extension under - PNP nomination

Hi All
I need your help. I got nominated back in December but I need to extend my WP. I'll be sending my PR application very soon.
My WP will expire in May so still got time. I know I won't have my AOR in time in order to apply to my BOWP extension.
I have two options apply for Employer-Specific WP or PGWP new policy. I'm going to do the first option.

Form IMM 5710.
I'm applying for "apply for a WP with the same employer"

Please help

My employer will submit the job offer under the Employer Portal
And I also need the following documents.
1. Application to change conditions, stay -- IMM 5710
2. Passport copy along with all visa/entry/departure stamps
3. Photo
4. Offer of Employment for a foreign National exempt LMIA
5. Employer Questionnaire. - I don't know what this one would be
6. PNP support letter (upload it under proof how you meet the requirements)
7. PNP nomination certificate
8. Employer payment receipt ($230 one)
