work permit at port of arrival

I am from country which has travel visa exemption (Portugal). I have positiv LMO and employment contract.
- I would first apply for a temporary work permit at the border port-of entry, do you need a medical exam, a police record or other documents before going to the border.What are the other documents I have to carry to produce to the immigration officers at the post of entry?
