Work Permit and TRV documents clarification

Hi,

I am planning to submit documents for my Work Permit and Spouse resident visa from outside Canada ( New York )

I was told to fill the below documents only .. is this correct ?

Principal Applicant
Document Checklist [IMM 5488]
Application for Work Permit Made Outside Canada [IMM 1295]
Family Information [IMM 5645]
Schedule 1 – Application for Temporary Resident Visa [IMM 5257 - Schedule 1]

Spouse
Family Information [IMM 5645]
Schedule 1 – Application for Temporary Resident Visa [IMM 5257 - Schedule 1]


Shouldnt I be filling the below for my spouse

Document Checklist [IMM 5484]
Application for Temporary Resident Visa [IMM 5257]
Family Information [IMM 5645]
Schedule 1 – Application for Temporary Resident Visa [IMM 5257 - Schedule 1]


Also, do you know how much they charge for the WP and Spouse .. Is it $ 150 for myself and $200 for my spouse , so total is $350 ?
