It sounds like they are getting health coverage before they are being approved for PR. They are just showing proof that they applied for PR, and that the sponsor has been approved.
I just called RAMQ (quebec health insurance) this only applies for inland application in my case unfortunately if not we need CSQ and PR approval plus immigration visa. If applying through vegreville once they say that you meet eligibilty to apply for PR than you can apply for provinvial health care and OHIP states the same info as RAMQ. I hope that there is a grey area!
