Why RICC request passport and medical examination at the same time?

After waiting for almost a year, my husband received responses from IRCC for his study permit. But he received correspondence letter, request for original passport, and medical examination at the same time. I suppose if IRCC requests original passport that means his study permit is approved. But how come they ask for medical examination then?
