I am sponsoring my 1 yr old kid. Can anyone please tell me whose name should I mention in page 4 of IMM0008 under "Consent to release information to intended province/territory of destination". Should I mention my ('am the father) name or should I mention my kid name. I know that the signature below would be mine, hence the confusion.

If the principal applicant is adult then obviously he would mention his name. But, in my case should i mention here my name or my kid's name.
I know I have to provide my signature on behalf of my kid. But, whose name to mention in above blank space.
