Whose information do I put on the online payment receipt??

I just paid for my application and ROPR fee online, and printed out several copies of the receipt. It has my boyfriends name on it, which I understand is not an issue (we used his credit card to apply). However, it then asks for date of birth, address, etc. Who am I putting there? Myself (the one immigrating to Canada)? There isn't a place to put my name as well so I'm a bit confused.

you can put either sponsor or applicant name and file# or UCI# if you have one.
