Dear All
I have receive ITA and I am gathering all my documents to submit with CIC
I have few questions and I am sure you all will help me.

I have lived in New Zealand for more than 6 months, please let me know where should I submit this •Completed Consent to Disclosure of Information (NZ) form with my Passport ID, all the documents will be uploaded online but where should I send this form with id? should I upload it as well or should I send it through postal mail( anyone who send it please help and guide)

secondly should I make PDF files for all my documents excluding Digital Photo or I can scan and upload documents in JPEG format too.

Help is badly needed so please help me.
