Where to add dependents on form IMM 0008

sorry for another question. my first application was returned for being incomplete. i didn't add dependents to form 0008 because they aren't coming and i also wrote a letter explaining such. but i dont think that was enough and they want the info anyway. so now im trying to fill out a pdf version (i can't add dependents after submitting unless i redo the entire application) anyway. the form only has room for one dependent and it says up to 4 on the instructions. so am i supposed to add them to the same box. or should i just fill out another form for each child? (there are two)
