Hi friends,
When Police clearance certificate and bank balance certificate is require in the process of express entry program ??
Please help me as soon as possible.
Bank certificate and bank statements are required twice - once at the time of submission of PNP application and second time during submission of e-APR.
There is no specific format for Bank certificate. Every bank has their own format.
The bank certificate should include - your account number, date of account opening, account holder name, current available balance and average balance for last 6 months.
Also, request the bank to provide you bank statement for last 6 months from the date of ordering. Ask them to stamp every page of that statement.
Again every bank has their own format for statements.
As sushsmart15 has mentioned, there is no specific bank certificate format. But I would suggest that you get a format drafted and take to the bank so that you get what you want on their letterhead, they may tweak it a bit, but as long as you have the required information, it should not matter.
You can modify the below format to suit your requirement. You can get the following on bank letter head.

I have received Balance Certificate from SBI on their letter head. Now my consultancy is asking me to provide Bank Statement also and in that all copies of bank statement must be attested by bank. Is attestation mandatory for bank statements as I already have Balance certificate?

Yes you need to get last 6 months bank statements from the bank and ask them to stamp every page. There is no need for attestation. Only make sure that the statements are stamped. You can print out the online statements and take them to your branch for stamping.
My bank is Govt. Bank..and these people are least cooperative...I am thinking of another option...If I take bank statement and notarize it, will it work?
You can ask the officials in polite manner, if still they're not cooperating, ask the branch manager for the complaint register.
No need to get it notarize, getting stamped statement from bank will be way easier. Also SBI has 'Customer Day' on 15th and 25th of every month.
