When does the 12 month clock start?

My file (spousal sponsorship for PR while living in Canada) currently has the following status

1. We received your application for permanent residence on January 22, 2019.
2. We started processing your application on November 28, 2019.
3. We sent you medical instructions on November 13, 2019. To avoid delays, please provide us the information requested in the letter as soon as possible. Please consider delays in mail delivery before contacting us.
4. We sent you correspondence on November 29, 2019. If you have not yet provided the information or the requested documents, please do so as soon as possible. Please wait until you receive the correspondence before sending us additional information, as the correspondence will outline all information that is required.
5. Medical results have been received.

First, judging from other threads, this seems like a really long time between application received and processing started. Maybe this is related to being in QC? My CSQ was issued in May.

But my main question is if the 12 month estimated processing time started last January or didn't start until November.

Mine was received January 7 and processing started October 29. Seems similar and normal. My ecas looks very similar. Mine is taking longer because mine went to Mexico though
