What should I mention? Base pay mentioned in the offer letter? Gross pay? Or something else?
What about joining bonus or any hike during the employment duration or any tax deductions?

It's more than perfect. They only his/her name and title. Contact information is for the company as a whole and you have provided that email where they can verify your claim.

Looks okay. The immediate supervisor's contact isn't necessary. it's HR's that's important.
My advise will be to get the letter dated around March 6th 2019, to ensure that there's no issue with the calculation of your 12 months of employment.
Ensure there's a company/HR stamp & (or) signature.
Also verify that your duties meet 80% of what it written in the NOC description that you're using.
Lastly, delete the link (your private information shouldn't be open to the public.
