What to upload as a proof of wife's legal name change?

Hello All

My wife had different birthname, and she changed her middle name to my name after marriage.

In express entry profile there are two place holder for name change document

If i make an affidavit for one and same person. can i upload same in two place holder ?

is it sufficient ?

I did this:

1) Letter of explanation
2) Attached - Marriage certificate
3) Attached - One & Same person notarised affidavit
4) Gazette of name change in India - This is not needed; but we had it so attached it.

I have gazette of name change along with other people who has been through name change on same day. I dont have anything for name change after marriage so i will make one affidavit for all of it.

One more thing did you get pr form express entry ? did you get any inquiry related to name change ?

I uploaded
1) one and same person affidavit from Canada stating this person had this names before marriage.
2) Marriage certificate
3) Birth certificate with previous name
4) Name change document obtained from Gov of Maharashtra, India

I got PPR

My wife's name was changed in passport after marriage. It is mentioned in passport that the new passport is issued with new name due to marriage and previous name was -------------.

Will that be sufficient as CIC is asking for legal documents in ITA application.
