What to fill in ORI filed in the FBI 258 form for PCC?

Hi Guys, I am filling in the FBI FD 258 form for my PCC in USA. (I am in India now) . I have a few questions for those who filled this form already, what should I mention in the following fields?

- ORI (this is mandatory) = ??
- Residence of person fingerprinted ( should I put a USA address or India address?)

I am planning to do a E request and elect for results by Mail

Any help with this would be very helpful.

applied on 5th feb, couriered the fbi card and got PCC on 27th feb

What about the signature of official taking fingerprint and Employer and address
