What should I put in the "Name of contact" and "Phone number" on question 11 as a business owner

As a business owner(self employer) whose company is only one person and one staff, what I should put in the "Name of contact" and "Phone number" on question 11.Can I put myself in the "Name of contact" and "Phone number" or my staff name or has to be other third-party persons?
And In "Occupation, field of employment or field of studies" field, should I put "Business Owner" or something else?
