I have a job reference letter 2 pages long telling all about my job responsibilities, roles and other intricate details. Do i need to have sign stamp and seal on all the pages?

Should attach my salary slips to it? If yes then can it be self attested or does it require gazzettation?
Should i attach my bank account statement and highlight the salary credit every month? can that be self attested or do we need to go to bank and do some affidavit for it?

should i mention the salary when i joined and the current salary?
should i write from "X" date to current date in the period of employment if i am still working with the same company i am requesting reference letter about!

Not necessary. Doesn't hurt if it is done.

Not mandatory. Can be attached. No need to notarize these.

Get it mentioned as 'started working with the organization on DD-MM-YYYY and is currently employed in XYZ capacity'.

Doesn't need to mention immigration but there's no harm done if it does. Doesn't need to recommend you for immigration - that will be plain silly and overdoing things.

1 reference letter per employer. You'll have to upload a copy in the designated placeholder in your MyCIC account.

Is there any other option? EE is all about soft copies and online uploads.

No. Unless they call you for an Interview. Keep a backup of everything that you upload.

One more silly question i want to ask if you dont mind. Is it true that only 50 occupations are included in FSW NOC categories and does it mean that if my noc code is not included in that then i wont be eligible for FSW?

My noc code is 0124 as i have been working for past 5 years in that field. So i have to practically help my employee draft the letter for me. my boss is actually very cooperative. :)
 
can u give any suggestions or links to such reference letters? or give some pointers like what to include to strengthen my chances (dont include FSW2014, i have already viewed them)

Follow exactly what CIC says and you should be fine.

Purpose

To prove each work experience claim and to validate that the applicant meets program requirements

Document requirements

The personalized document checklist in MyCIC prompts the applicant with an upload field for each work experience declared. At completeness check, the processing office only determines whether the required documents are included. The validity or fraudulence of the information is assessed only if and when the application is placed into processing. The following documents are mandatory for each work experience declared:

A reference or experience letter from the employer

should be an official document printed on company letterhead (must include the applicant’s name, the company’s contact information [address, telephone number and e-mail address], and the name, title and signature of the immediate supervisor or personnel officer at the company);

should indicate all positions held while employed at the company and must include the following details: job title, duties and responsibilities, job status (if current job), dates worked for the company, number of work hours per week and annual salary plus benefits.

If the work experience is in Canada, proof may include copies of T4 tax information slips, Notices of Assessment (NOA) and Option C printouts issued by the Canada Revenue Agency (the time period for these documents should reflect the work experience timeframe, e.g., work experience from 2006 to 2008 requires only documents from those calendar years).

If the applicant is self-employed, articles of incorporation or other evidence of business ownership, evidence of self-employment income, and documentation from third-party individuals indicating the service provided along with payment details (self-declared main duties or affidavits are not acceptable proof of self-employed work experience).

Individuals who must submit this documentation

The principal applicant
Their spouse or common-law partner (if work experience in Canada is claimed)

Possible but not in 100% cases. Only if the VO has any doubts about your document(s).
 
I don't have any ties in Ontario. Should I still document anything and tick the box "Ties to Ontario" in the form?
