I have PR but did not complete or satisfy the RO as my mom was ill. What documents is needed to show illness of my mother so I can claim H&C grounds?

You'll want things like hospitalization records, letter from doctors confirming she needed assistance / wasn't able to live independently, evidence that you were the only family member available to take care of her (i.e. you have no sibilings living in your home country), etc.
