1. Degree Certificate
2. Semester wise Mark sheets

Should I also send the 'Academic Records Request Form' that I used to get the transcripts from my university ?
I've made a mistake. I have used the form for USA instead of Canada to get the transcripts from my university. Please let me know if this document should also be sent.
