Hi All,

I am about to fill this form for WES verificationbut not sure about 2 column i.e. Last/Family Name and Given name
My name in passport is Amandeep Kaur Brar but in my academic records (all degrees) it is Amandeep Kaur.
So now in this form in the last and given name shall I put the name given in Passport and in previous name my degree name??

please help me on this.
