I thought I should ask this question here because people might be having greater knowledge on application forms etc.,

Applied for PR-FSW thru express entry and I got a passport request yesterday.
With this I was requested to upload "kit, application forms".
I have no idea what is this form.
Does anyone know this?
Currently I am working in Canada with work permit status and I have less than 1yr experience. I was invited to apply under FSW in Express entry.
