Visitor Visa Passport Request Help

Hi,

I applied for a visitor visa and finally got the passport request letter and have a few questions.

Is it best to send it to LA or NYC (I am closer to NYC)?

Is the following all I need to include? I also saw another site with a lot of information it had another list of documents, so I'm not sure.
The passport request letter you received from CIC
Any applicable VAC fees, such as passport transmission fees (is it $28.35 or $33.45? That goes to a lockbox somewhere else?)
Full contact information, including a phone number and your full return address
The VAC consent form (PDF, 18 KB)
A pre-paid return courier envelope, if applicable (do I need this as well???)

Do I need to include anything else?

HELLO MY WIFE GOT PASSPORT REQUEST TOO.....

SHE IS CLOSER TO NYC OFFICE ...

SHE IS GOING TO MAIL HER PASSPORT TO NYC BUT DOESN'T KNOW HOW TO PAY $28.35 USD VAC FEES ???

NEED HELP
