My question is should be prepare a letter of explanation.

There is no such thing as a "visitor visa"... most likely you are referring to a TRV (Temporary Resident Visa).
As your friend is from a visa-exempt country she is not eligible to have any kind of visa. "Visas" cannot be extended.
Your friend wants to extend the "visitor status" that she got when entering Canada. She will have to apply for "extend my stay as a visitor" regardless on whether she is from a visa-exempt country or not if her authorized stay is close to over.
