Im applying for visitor visa extension for my mother who is in Canada now. Does she need medical examination? anyone knows?

1. It depends. If she has been resident in a scheduled country see: http://www.cic.gc.ca/english/information/medical/medexams-temp.asp
2. If she is from one of the listed countries and she intends to be in Canada for more than 6 months, then she will need a medical.
3. If you apply for the extension, without the medical, then she will only be given 30 days to have the medical.

1. As long as her total time in Canada is less than 6 months with the extension, then she doesn't require a medical.

Do i need to write a new invitation letter and have it notarized again for visa extension?

And also need to translate my birth certificate again and have that notarized as well?
