Visitor visa extension and job offer

Hi my fiancé has recieved a job offer and has lmia exempt number and has a 3 month visitor visa and we will apply for extension should we include work permit application and job offer in the extension application? Or should we just apply for work permit? What if we dont have lmia submission or confirmation letter? Is there an lmia exempt submission letter or confirmation letter? And will i need to include it with the application? Also can we just flagpole? Its kind of urgent
