hi,

1. I have received call for medicals which is fine but family name in my medical form has been wrongly entered as "Bhandari Rajesh" where as it should only be "Bhandari" AND the given name is infact" rajesh" which has been correctly mentioned as "rajesh".

what shall I do -go ahead with the medicals or ask the immigration section at visa office new delhi to correct the family name.

2. I have also been asked to deposit the fee. though I do have master card or visa debit card but they are not international cards and the payment is to be done in CADs. can I use some body else's credit card.-and what is the other available method of paying the fees

3. they have given a post box address. what post shall I use I don't suppose currier service delivers on P.O. BOX addresses.

My personal opinion is that your panel Physician would mention correct particulars as per the Passport in the eMedical forms.
 
yeah.....but the form has already been supplied by them where in they have put in the particulars before hand.

The form you got is a pdf file while the Doctor ( or the front desk Clerk ) would enter the details 'online' in real time.
 
I don't think u r talkin india. the doctor in the panel in my part of the world is not e-enabled.
